Treatment of granulomas
Granuloma is one of the most common complications of tooth decay and pulpitis. It is the escape of the infection outside the tooth with the formation of a capsulated sac filled with dead cells. For a long time, the development of granuloma can proceed without pronounced symptoms. As it develops, the disease manifests itself through suppuration and encapsulation of the granuloma, i.e. formation of a pouch with purulent contents in the gum (near the root of the tooth).Symptoms of granuloma
Externally, the development of this subtype of cyst is manifested as follows:- a gingival tumour;
- purulent discharge;
- minor pain that worsens when biting or chewing food;
- discolouration of the enamel of the affected tooth.

Why does a granuloma appear
The main causes of granuloma and cysts are:- pulpitis;
- substandard treatment.
- Suffered dental trauma – infection can penetrate broken teeth;
- the development of certain colds and inflammatory diseases, such as tonsillitis.
Diagnosis
If a granuloma is suspected, the doctor will send the patient for an X-ray. You can find out how much this service costs on the “Price” page. On the image, the formation will be reflected by darkening in the problem area. After that, it is possible to refer for a targeted dental X-ray to make an accurate diagnosis, determine the size of the granuloma and develop treatment tactics. It should also be differentiated from other forms of periodontitis, such as cysts. Unlike a granuloma, a cyst may be larger in size, but may not show any symptoms for longer.
Types of granuloma
Depending on the place of localisation, specialists distinguish the following types of formation:
- Apex – occurs most often, in this case the apex of the root of the tooth is inflamed;
- Root granuloma – develops near the root of the tooth;
- Intercorneal – between the roots of the teeth;
- Apical granuloma – localised near the apical opening of the tooth.
Today, all of these subtypes of the disease are effectively treated using a number of standardised regimens.
How it’s treated
At the Dent-House Clinic, specialists use both conservative and surgical methods to treat granuloma. Depending on the initial condition of the tooth, the treatment plan may differ slightly. If the tooth canals are filled, the treatment is preceded by surgery to unseal the canals. If the root canals are not filled, treatment involves the following steps:
- removal of carious tissue;
- aseptic canal treatment;
- filling.
If the granuloma is up to 3 mm in diameter, a complete filling is possible. If the capsule is larger, a temporary paste based on calcium hydroxide is used. After a few weeks, an X-ray is scheduled, and if there is no inflammation, a gutta-percha filling is made and the tooth is subsequently restored. In some cases, surgical treatment is performed:
- for root canal obstruction;
- if it can’t be unsealed;
- if there is a pin in the canal;
- If the patient does not want to remove the artificial crown.
In these cases, the granuloma is removed. Depending on the situation, the apex is resected or hemi-sectioned, i.e. one of the roots with part of the crown of the tooth is removed. In such case, there is a need for subsequent prosthetics. If purulent complications develop in the periosteum or in the oral mucosa, the pathological formation is opened to drain its contents. After that, a course of antibiotics will be prescribed to suppress the possible spread of infection. It should be understood that independent cure of granuloma is impossible. For its elimination, dental care is necessary.
